Services offered at this location:

Our team helps patients learn coping skills and self-care. We work together to find the best treatment path for patients. Our outpatient care means you get care while living at home and continuing to work or go to school and spending time with family and friends.

If patients need continued medical care after they leave the hospital, we can help. “Post-acute” care means services that support your recovery. These services can happen in your home or in a skilled nursing facility.

Our Senior Health program provides education, support, and care in older age. We focus on the needs of our patients and their families with many types of support. Our Fall Prevention Center helps with skills and ideas to prevent falls.

A skilled nursing facility is for patients who need a lot of help after an illness or injury. We provide many types of services while patients stay with us. Our Acute Rehabilitation Hospital treats most of Alameda County’s traumatic brain injuries.

Subacute care is for patients who need special, long-term services but do not need to stay in the hospital. Our care team will work with patients on the best personal care plan.
